Instructions:

1. Click the application link below.
2. Fill out infomation as completely as possible. Some fields on the application are not enterable, such as dates and signatures, which will be required upon your interview. Any incomplete or incorrect areas on the application can be completed or corrected upon your interview. In order to preview and retain a copy of the application for your records, click on the save and preview button once you filled out all pages. You will need Adobe Acrobat reader (click here for download) to read a saved application.
3. When satisfied, click on the submit application button. You should receive confirmation from us of receipt via email.

Applications can also be faxed to the Pleasant Hill Branch at 925-930-7130 or the Emeryville Branch at
510-923-1427. In most cases, upon receipt of you application, we will contact you within 3 business to set up an interview if we have not heard from you.
